Home Care Services
Home Care Services provides personal care assistance to veterans, adults, and children. The service also provides Respite and Sitter services as well. A Certified Nursing Assistant and/or a Personal Care Assistant serve as a resource to those needing support with their daily living requirements.
Provides a package of services to allow adults (age 18 and older) who qualify for nursing facility care to remain in their private residences and receive personal care services. This service includes support, supervision, and engaging participation with eating, bathing, dressing, personal hygiene, and other daily living activities.
Provides service for medically fragile children (through age 18) who would otherwise require long-term hospital care or nursing facility care. The program contributes to the quality of life for children and their families/caregivers. This service includes support, supervision, and engaging participation with eating, bathing, dressing, personal hygiene, and other daily living activities.
It is a service that provides periodic relief for the family or primary caregiver as detailed in the approved Person-Centered Plan. This service is in the participant’s home or an out-of-home setting.
The Program All-Inclusive Care for the Elderly(PACE) program model is centered on the belief that the well-being of the seniors with chronic care needs and their families to be served in the community/home whenever possible. Rainbow 66 Storehouse has partnered with these entities in the Greensboro area.
Mental Health Services
(North Carolina Innovations Waiver)
Rainbow 66 Storehouse services provide assistance with acquisition, retention, and/or improvement in skills related to activities of daily living. Those services include:
PRIMARY CRISIS SERVICES
It provides an additional staff person trained in behavior techniques, participant’s person-centered plan, and the crisis plan. It offers services to the participant, as needed, during an acute crisis to continue in his/her daily routine and or residential setting without interruption.
Provides assistance with choosing/matching, acquiring/finding, development, and initial job training for participants ages 16 and older for whom competitive employment has not been achieved and or has been interrupted or intermittent. Services include Pre-job training/education and development activities, including career/educational counseling, job shadowing, assistance in the use of educational resources, training in resume preparation, job interview skills, study skills, etc., for job retention. The services may also include assisting a participant in developing and operating a micro-Enterprise or small business.
RESPITE (b)(3) Services
Provides periodic support and relief to the primary caregiver(s) from the responsibility and stress of caring for children ages three(3) to twenty-one (21) with mental health, developmental disabilities, or substance use/addiction service needs, and for adults 21 and over with developmental disabilities.
LONG TERM VOCATIONAL SUPPORTS
Assist with maintaining a job for participants, age 16 and over, who no longer need the intense level of Supported Employment Services. Long Term Vocational Support supports the participant who, because of specific needs of the participant, including medical and/or behavioral needs, may require support regarding the social or environmental issues that are a part of successful employment. This service is intended to provide supports to the participant, who has successfully developed the skills to complete the identified job.
NATURAL SUPPORTS EDUCATION
Provides training to families and the beneficiary’s natural support network to enhance the decision-making capacity of the natural support network, provide orientation regarding the nature and impact of the intellectual and other developmental disabilities upon the beneficiary, provide education and training on intervention/strategies, and provide education and training in the use of specialized equipment and supplies.
It assists with acquisition, retention, or improvement in daily living activities, such as personal grooming and cleanliness, bed making and household chores, eating and preparing food, and the social and adaptive skills necessary to enable the individual to reside in a non-institutional setting. To reflect the natural flow of training, the practice of skills, and other activities as they occur during a person’s day, training and instruction are together with support, supervision, and participation.
Day Support services are provided to adults with intellectual/developmental disabilities. Participants in our Durham and Scotland counties enjoy a week full of productive activities meant to stimulate creativity. Exercise, arts and crafts, music, etc., are encouraged here, along with continuing education. In Durham, NC, we serve as a location for Durham Technical College, which offers Adult Basic Education classes weekly to the participants involved in this program.
COMMUNITY LIVING & SUPPORT
Enables the person to learn new skills, practice, and/or improve existing skills. Areas of skill acquisition are interpersonal, independent living, community living, self-care, and self-determination.
Provide individualized day activities that support the waiver beneficiary’s definition of a meaningful day in an integrated community setting with persons who are not disabled. The services consist of participation in adult education, development of community-based time management skills; volunteer work; using public transportation, community-based classes for the development of hobbies or leisure/cultural interests, etc.
Promote self-determination, support the individual in making life choices, provide advocacy and identify opportunities to become a part of their community.
COMMUNITY GUIDE (b)(3) WAIVER
Provides support to the beneficiaries and planning teams that assist beneficiaries in developing social networks and connections within local communities. This service aims to promote self-determination, increase independence, and enhance the beneficiary’s ability to interact with and contribute to his or her local community.
